Factors Affecting Psychiatric Disorders in Offspring of Parents with Bipolar Disorder

Abstract

|
Objectives : The offspring of bipolar parents are vulnerable to developing affective and other psychiatric disorders. The identification of demographic and clinical factors for psychiatric disorders among the offspring of bipolar adults could improve preventive and treatment strategies. The aim of this study was to investigate the risk factors of mental disorders in the offspring of bipolar parents.
Methods : Subjects included 148 child and adolescent offspring (aged 6.0 yr to 18.9 yr) from 105 nuclear families with at least one parent with bipolar disorder (BPD). Probands, offspring, and biological co-parents were interviewed using a semi-structured diagnostic interview, and the offspring were evaluated using the Korean version of the Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version (K-SADS-PL) and Korean Childhood Trauma Questionnaire (K-CTQ).
Results : The offspring with mental disorders were older, had higher K-CTQ scores, and their mothers were younger at the time of their birth, while the offspring diagnosed with BPD were also older and had higher rates of comorbid disorders (anxiety disorders, ADHD, and disruptive behavior disorder); furthermore, mental disorders were more prevalent among the non-bipolar parent. In the multivariate logistic regression, the variables that are predictive of psychiatric disorders are older offspring age, younger mother age at birth, and higher K-CTQ Score; for the BPD in the regression analysis, the associated variables are older offspring age and higher rates of comorbid disorders.
Conclusion : The offspring of bipolar parents are at a high risk regarding mental disorders; furthermore, these findings could have implications for the diagnosis and treatment of psychopathology among the offspring of bipolar parents.
